1. Who we are
- Family Art IVF.
- This policy covers the familyartivf.com website and the Family Art IVF team. Family Art IVF is responsible for your personal data (the data controller). In this policy, "we", "us" and "our" mean Family Art IVF.
- For anything about your personal data, write to [email protected].
- We apply the standards of the General Data Protection Regulation (GDPR) to all personal data we handle, wherever you live, together with the data protection laws of Azerbaijan and Cyprus, where your treatment takes place. For people living in the United Kingdom, the GDPR applies as the UK GDPR.
2. What we collect
- When you ask for a call back: your name, phone number with country code, your email address if you choose to give it, and your consent to be contacted.
- With the same request, information about how you reached us: the page where you first arrived, the referring website, campaign tags in the web address (UTM parameters) and advertising click identifiers (gclid, fbclid). These contain no health information.
- When we talk to you and plan your treatment: information you give us about your situation, which may include your medical history, test results, scan reports and those of your partner.
- To arrange your travel and stay: your travel dates and the names of the people travelling with you.
- Technical data: to protect the form against abuse, your IP address is turned into a one-way code and kept for one hour. We do not store your IP address with your request.
- Cookies and similar technologies, as described in our cookie policy. Advertising and analytics cookies are used only with your consent.
- We do not ask for health information on the website form.
3. How we use your information, and why we are allowed to
- To call you back and answer your questions: because you asked us to and gave your consent.
- To plan and organise your treatment, appointments, stay and transfers: because it is needed for the services you ask for, before and under our agreement with you.
- To keep records required by law, including medical and financial records: because the law requires it.
- To know which advert or page brought you to us, so we can measure our advertising: with your consent, given through the cookie banner. Only one event is sent to advertising platforms when a request is sent, and it carries no form content, no treatment and no health information.
- To keep the website and our systems secure and to prevent misuse: because it is in our legitimate interest and in yours.
- We do not make decisions about you by automated means alone.
4. Health information
- Information about your health, fertility and treatment is sensitive data and receives extra protection.
- We use it only with your explicit consent and only to plan, carry out and follow up your treatment. Access is limited to the people who need it for your care.
- You can withdraw your consent at any time by writing to us. This does not affect what was done before, and it may mean we can no longer arrange your treatment.
5. Who we share it with
- The medical team at DNA Clinic in Baku, Azerbaijan, to plan and carry out your treatment.
- Caspian International Hospital in Baku, where procedures take place.
- Miracle Hospital in Nicosia, Cyprus, if you have treatment there, to plan and carry out your treatment.
- MotherScan, our partner clinic in London, when you book tests and follow-up scans there. MotherScan is responsible for the data it holds about the services it provides to you.
- CityLife Boutique Hotel and our transfer drivers in Baku and Cyprus: only your name and travel details, never health information.
- Companies that provide services to us, such as website hosting, email delivery and Cloudflare, which protects the site and checks that the call-back form is sent by a person (Cloudflare Turnstile). They act only on our instructions and may not use your data for their own purposes.
- With your consent only: Google and Meta, which receive the advertising and analytics data described in our cookie policy.
- Public authorities or courts, where the law requires it.
- We never sell your personal data.
6. Transfers between countries
- Your treatment takes place in Azerbaijan or Cyprus and your tests and follow-up may take place in the United Kingdom, so your data is shared between those places.
- Data sent from the United Kingdom to Azerbaijan or Cyprus is transferred with your explicit consent, given when you agree to your treatment plan, and with contractual protections between the parties involved.
- Some service providers may store data in other countries. Where they do, we use contractual protections that keep your data safe to the same standard.
7. How long we keep it
- Call-back requests that do not lead to treatment: up to 24 months after our last contact, then deleted.
- Records of patients we have helped, including correspondence and invoices: 6 years after our last contact.
- Medical records held by the clinic that treats you: for the period required by the law of Azerbaijan or Cyprus.
- The coded IP address used to protect the form: one hour.
- Cookies and browser storage: for the periods listed in our cookie policy.
8. How we protect it
- The website uses an encrypted connection (HTTPS). Access to your information is limited to the people who need it, and they are bound by confidentiality.
- We ask you not to send medical documents by ordinary email unless we have agreed this with you.
9. Your rights
- You can ask to see the personal data we hold about you and to receive a copy.
- You can ask us to correct data that is wrong or incomplete.
- You can ask us to delete your data, unless the law requires us to keep it.
- You can ask us to limit how we use your data, or object to its use.
- You can ask for the data you gave us in a format you can pass to someone else.
- You can withdraw your consent at any time, including your consent to advertising cookies.
- To use any of these rights, write to [email protected]. We reply within one month. We may ask you to confirm your identity first.
10. Complaints
- If you are unhappy with how we handle your data, please tell us first at [email protected] so we can put it right.
- You can also complain to the data protection authority in the country where you live or work, or where you think the problem happened. In the United Kingdom this is the Information Commissioner’s Office (ico.org.uk); in Azerbaijan and Cyprus, the authorities responsible for personal data.
11. Children
- This website and our services are for adults. We do not knowingly collect personal data from anyone under 18.
12. Changes to this policy
- We may update this policy when our services or the law change. The date at the top shows the current version. If a change affects you significantly, we will tell you.
